What happens next

Send the form or call the office at (664) 104-9153.
You are told which imaging and documents to bring, and given an appointment.
At the visit: clinical history, neurological examination and imaging review on screen.
You leave with a diagnosis, the alternatives and a written plan — with or without surgery.
If surgery is indicated, scheduling, hospital and pre-operative work-up are arranged.
